1

Remove container from packaging and place at point of use. Do not remove absorbent pad from container.

2

Save the return shipping box that already has the pre-paid return label attached, as well as the associated components (plastic bag, twist tie, and tape). This is the box you'll use to return your container.

3

After each infusion, place sharps and other biohazardous materials into the container. Do not overfill. (Hint: biohazardous material is anything that came into contact with your bodily fluids)

4

Close lid by snapping firmly over opening. Make sure lid is tightly sealed prior to packaging.

5

When ready to return a container, place it into the plastic bag and then into the return shipping box. Only mail container in original return box.

6

Securely seal the bag around the container using the included twist-tie.

7

Close the numbered shipping box flaps in order. The Return Label must be showing.

8

Place the included tape over closer flap as pictured above.

9

Remove Tracking Form from plastic pouch on shipping box. Complete top section. You must sign the form. Remove bottom copy and keep. Place remaining copies back in pouch with barcode facing out. Box will not be accepted without tracking form.

10

Fill out your return to sender address in the area provided on top of the return box and then give it to your USPS mail carrier, drop it off at a USPS office or request a USPS pickup. No additional postage is necessary. Total package may not weigh more than the weight listed on the shipping box.

That's it! Once your box is processed at the recycling center, patients who are IgCares Support Program members will earn 1 donation point.

We'll also automatically send you a new Sharps container and the associated return box with pre-paid USPS shipping label.

In case of damage to the Sharps container or for general assistance, please call 800-772-5657.

5

Place the bagged foam cooler, itemized list of contents (and any other appropriate materials) into the second 4-mil large plastic bag, securely sealing everything with the second tie wrap. Place the double-bagged cooler into the prepaid, prelabeled UPS Ground–only shipping box.

6

Close the shipping box flaps, leaving “Return Shipping Panel” exposed. The ship-to label must be showing.

7

Tape the shipping box closed with the included packaging tape, making sure that the UPS prepaid return shipping label is visible.
