1. General

These Terms (as defined below) are published on the IgCares Program website (“Site”) at igcares.com.

The IgCares program (the “Program”) is a voluntary program administered by Octapharma USA, Inc. and its affiliates, business partners and licensors (“Sponsor”). When participants in the Program engage in certain activities on their journey to a healthier lifestyle, they can collect points, which may be donated to listed non-profit organizations (the “Program Charities”). The Program is not a sweepstakes or contest. Participation in the Program is void where prohibited by law.

These Terms constitute a binding contract between yourself, visitors or users of the Site (“user,” “you” or “Members”) and Sponsor. By accessing, browsing, or using this Site or the content provided to you through the Site, you acknowledge that you have read, understood, accept, and agree to be bound by and comply with these terms, which hereby incorporates additional terms, found here: Octapharma Website Terms of Use as well as the practices described in our Privacy Policy, found here: Privacy Policy ( and are collectively the “Terms”) and all applicable laws and regulations. Terms may be updated periodiclaly without notice; you are advised to revisit these Terms to learn of any changes. If you do not agree to be bound by these Terms, please do not use the Site or enroll in the Program.

2. Eligibility and Registration

Participants in the Program (“Members”) must be (a) residents of the United States, (b) at least 18 years of age at the time of registration, and (c) must register at igcares.com or through the IgCares; mobile app. Individuals do not need to be taking cutaquig® in order to become Members.

3. How to Participate

Following registration, Members may participate in the Program as follows:

(a) Program Activities
Members may receive points by participating in various activities, for example, reading or watching educational material, receiving and scanning a cutaquig prescription or refill, or participating in the infusion materials recycling program (“Program Activities”). The number of points collected by completing a particular Program Activity may vary depending on the activity, and will be disclosed at the time the Member participates in the activity. Program Activities may be accessed and completed at igcares.com, or through the IgCares mobile app.

(b) Donation Activities
After collecting points, Members may allocate them for donation to one of the Program Charities. Sponsor will donate one U.S. dollar (US$1.00) (the “Point Donation Amount”) to a Program Charity for each point donated up to a collective maximum annual dollar amount (the “Maximum Annual Donation”) set forth in the Program Limitations below.

4. Program Limitations

(a) Points
Program points have no cash value to Members and cannot be redeemed for any purpose other than donation through the Program. All points collected must be donated by Members during the current 12-month calendar period. Points collected but not donated by a Member during a 12-month calendar period will expire and no longer be valid for donation.

(b) Maximum Annual Donation to Program Charities
The Maximum Annual Donation to the Program Charities will be $150,000, which may include donations made by Sponsor for IgCares activity as well as other Sponsor contributions outside of the IgCares program.

(a) The Immune Deficiency Foundation (“IDF”)

(c) The Foundation for Primary Immunodeficiency Diseases (“FPID”)
The Foundation for Primary Immunodeficiency Diseases (FPID), supporting PI education, early diagnosis, counseling, therapy, and research. Visit fpid.org

(d) International Patient Organization for Primary Immunodeficiencies (“IPOPI")
IPOPI’s mission is to improve awareness, access to early diagnosis and optimal treatments for PID patients worldwide. Visit ipopi.org

6. Privacy and Data Use

Sponsor respects the individual privacy and values the confidence of each of our Members. The information that we collect during Member registration will be used solely for the purpose of administering the IgCares Program and as otherwise permitted in our Privacy Statement. By joining the Program, Members agree that Sponsor may send them communications related to the Program, including site updates, education, and general information.

7. Program Changes and/or Termination

Sponsor reserves the right to amend or terminate the Program, including any Program-related Services, website or mobile app, at any time without notice, including, without limitation, amending these Terms and Conditions.

8. Prohibited Activities

This site is for private, internal, non-commercial use only. You are prohibited from any other use, including modifying, copying, distributing, transmitting, displaying, reproducing, publishing, selling, licensing, transferring, assigning, reverse engineering, editing, adapting, or creating derivative works from the Site. In addition, you may not provide any information that is untrue, inaccurate, or misleading to this Site, nor violate any restrictions or bypass measures that prevent or limit access to this Site. Furthermore, you are prohibited from interfering with the proper working of the Site or any activities conducted on the Site or using the Site in any manner that could damage, disable, overburden, or impair the Site or interfere with any other party's use and enjoyment of the Site. Unauthorized access to the Site, other accounts, computer systems or networks connected to the Site through hacking, password mining or any other means, or attempting to obtain any materials or information through any means not intentionally made available through the Site is/are strictly prohibited.

9. Notification of Unauthorized Use/Ceasing Access

You shall notify Sponsor immediately of any unauthorized use or threat of unauthorized use of your account or the Site or of any other breach or potential breach of security known to you with respect to your account or the Site, including without limitation any loss or compromise of any password, and You agree to cooperate to help Sponsor prevent the further unauthorized use, threat of unauthorized use, disclosure or threat of disclosure regarding the Site, your account and/or your password. You agree that immediately upon termination of your right to use the Site, or upon any earlier demand by Sponsor at any time, you will cease all access and/or use of the Site, and will no longer attempt to access and/or use the Site.

10. Confidentiality of Account and Password

You are solely responsible for maintaining the confidentiality of your account and password and for restricting access to your computer. You accept full responsibility and liability for all activities that occur under your account or password. Sponsor reserves the right to refuse service, terminate accounts, or remove content in its sole discretion. The information that you supply during any registration process must be accurate and complete and you agree to not (1) register under the name of another person, (2) choose a user name that Sponsor deems offensive, or (3) choose a user name for the purposes of deceiving or misleading anyone as to your true identity.

11. No Warranty; Disclaimer of Liability

There is no assurance that the Site will be or remain uninterrupted, that the content or any other content posted or uploaded by third parties will fulfill any of your particular purposes or needs, that Content will in all cases be non-infringing of a third party’s rights, that defects of any kind will be corrected, or that the products or services offered through the Site are or will remain available in all jurisdictions. SPONSOR SHALL NOT BE LIABLE FOR ANY OF THE FOLLOWING TO INCLUDE: DIRECT, INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR DAMAGES FOR COST OF PROCUREMENT OF SUBSTITUTE PRODUCTS OR SERVICES, LOSS OF PROFIT, REVENUE, DATA, GOODWILL OR USE INCURRED BY YOU OR ANY THIRD PARTY, ARISING OUT OF, OR CONNECTED WITH (A) ANY USE, BROWSING OR DOWNLOADING OF ANY PART OF THE SITE OR CONTENT; (B) ANY DEFECT, OMISSION, ERROR, INTERRUPTION, OR VIRUS, IN ANY CONTENT, PRODUCTS, OR SERVICES OBTAINED THROUGH THE SITE, OR ANY SUBMISSION BY ANOTHER USER POSTED ON OR AVAILABLE THROUGH THE SITE; OR (C) ANY INABILITY TO USE, DELAY IN USE, OR OTHER FAILURE OF ANY COMPONENT OF THIS SITE FOR ANY PURPOSE.

If, despite the limitation above, Sponsor is found liable for any loss or damage in any way connected with the Site or the occurrences described in the limitation above, then the liability of Sponsor will in no event exceed, in the aggregate, the greater of (a) the service fees, if any, that you paid to Sponsor in connection with the service that you received from this Site, or (b) ten dollars (US$10.00). Some jurisdictions do not allow the limitation of liability, thus the limitations above may not apply to you.

Termination

Sponsor may, in its sole discretion, any time and without advance notice or liability, terminate, suspend, restrict or block your access to all or any component of the Site, even if access continues to be allowed to others. If you breach any of the provisions of these Terms, Sponsor, in its sole discretion, may delete anything you have submitted or posted on the Site. Sponsor shall not be liable to you for such actions, including termination, suspension, restriction, or blocking. Any suspected fraudulent, abusive, or illegal activity that may be grounds for termination of your use of the Site may also be referred to appropriate law enforcement authorities. These remedies are in addition to any other remedies that we may have at law or in equity.

Upon such termination, suspension, restriction, or blocking, you must immediately discontinue use of the Site, and destroy any copy you have made of any portion of the Site. Accessing the Site after such termination, suspension, restriction, or blocking shall constitute an act of trespass. We shall not be responsible to you for such termination, suspension, restriction or blocking.

Any cause of action arising out of, or related to, the Site must commence within one (1) year after the cause of action accrues. Otherwise, such cause of action is permanently barred.

Notices

Written Documents
You may preserve these Terms in written form by printing them for your records, and you waive any other requirement that these Terms be evidenced by a written document. A printed version of these Terms and of any notice given in electronic form shall be admissible in judicial or administrative proceedings based upon or relating to these Terms to the same extent and subject to the same conditions as other business documents and records originally generated and maintained in printed form.

Electronic Notifications
To the extent that we may need to contact you, you agree that we may do so via any electronic means, including but not limited to communication posted on the Site, electronic mail, or mobile SMS messaging.

Miscellaneous

No Waiver
If we fail to act with respect to your breach or anyone else's breach on any occasion, we are not waiving our right to act with respect to future or similar breaches.

Assignments
We may assign our rights and obligations under these Terms to a third party without this assignment being deemed a change in these Terms, and without notice to you. You may not assign, delegate, or transfer your rights or obligations under these Terms.

Entire Agreement
These Terms (including the, Octapharma USA Website Terms of Use, and Privacy Policy constitute the entire agreement between you and Sponsor with respect to the Site. They supersede all prior or contemporaneous communications and proposals, whether electronic, oral, or written, between you and Sponsor with respect to the Site.

Reservation of Rights
Any rights not expressly granted herein are reserved.

Government Access to Data
Our performance under these Terms is subject to existing laws and legal process. Sponsor will comply with law enforcement requests or requirements relating to your use of the Site or information provided to, or gathered by Sponsor with respect to such use.

Thrombosis may occur with immune globulin products, including cutaquig.

Risk factors may include: advanced age, prolonged immobilization, hypercoagulable conditions, history of venous or arterial thrombosis, use of estrogens, indwelling vascular catheters, hyperviscosity, and cardiovascular risk factors.

For patients at risk of thrombosis, administer cutaquig at the minimum dose and infusion rate practicable. Ensure adequate hydration in patients before administration. Monitor for signs and symptoms of thrombosis and assess blood viscosity in patients at risk of hyperviscosity.

Indications and Usage

Cutaquig® (Immune Globulin Subcutaneous (Human)-hipp) is a 16.5% immune globulin solution for subcutaneous infusion (IGSC), indicated as replacement therapy for primary humoral immunodeficiency (PI) in adults and pediatric patients 2 years of age and older. This includes, but is not limited to, common variable immunodeficiency (CVID), X-linked agammaglobulinemia, congenital agammaglobulinemia, Wiskott-Aldrich syndrome, and severe combined immunodeficiencies.

IMPORTANT SAFETY INFORMATION

Contraindications

Cutaquig is contraindicated in patients who have had an anaphylactic or severe systemic reaction to the subcutaneous administration of human immune globulin or to any of the components of cutaquig such as Polysorbate 80, and in IgA-deficient patients with antibodies against IgA and a history of hypersensitivity.

Warnings and Precautions

Severe hypersensitivity reactions may occur with cutaquig, even in patients who tolerated previous treatment with human immune globulin. If a hypersensitivity reaction occurs, discontinue the cutaquig infusion immediately and initiate appropriate treatment. IgA-deficient patients with anti-IgA antibodies are at greater risk of severe reactions.

Thrombosis may occur following treatment with immune globulin products, including cutaquig. For patients at risk of thrombosis, administer cutaquig at the minimum dose and infusion rate practicable. Ensure adequate hydration in patients before administration. Monitor for signs and symptoms of thrombosis and assess blood viscosity in patients at risk for hyperviscosity.

Falsely elevated blood glucose readings may occur during and after the infusion of cutaquig with some glucometer and test strip systems. When administering cutaquig, measure blood glucose with a glucose-specific method.

Aseptic meningitis syndrome (AMS) can occur with cutaquig. AMS has been reported after the use of human immune globulin administered intravenously and subcutaneously and may occur within 2 days following treatment. Discontinuation of immunoglobulin treatment has resulted in remission within several days without sequelae.

Acute renal dysfunction/failure, acute tubular necrosis, proximal tubular nephropathy, osmotic nephrosis and death may occur with use of human immune globulin, especially those containing sucrose. cutaquig does not contain sucrose. Monitor patients for signs and symptoms of renal dysfunction. Monitor blood urea nitrogen, serum creatinine, and urine output in patients at risk of acute renal failure.

Monitor cutaquig recipients for clinical signs and symptoms of hemolysis, particularly patients with pre-existing anemia and/or cardiovascular or pulmonary compromise. Consider appropriate confirmatory laboratory testing if signs and symptoms of hemolysis are present after cutaquig infusion.

Non-cardiogenic pulmonary edema may occur in patients administered human immune globulin products. Monitor for pulmonary adverse reactions (transfusion-related acute lung injury, TRALI). If TRALI is suspected, perform appropriate tests for the presence of anti-neutrophil antibodies in both the product and patient’s serum. Patients with TRALI may be managed using oxygen therapy with adequate ventilatory support.

Cutaquig is made from human plasma and may carry a risk of transmitting infectious agents, e.g. viruses, the variant Creutzfeldt-Jakob disease (vCJD) agent, and, theoretically, the Creutzfeldt-Jakob disease (CJD) agent.

Drug Interactions

After infusion of cutaquig, the transitory rise of the various passively transferred antibodies in the patient’s blood may yield false positive serological test results, with the potential for misleading interpretation.

Adverse Reactions

The most common adverse reactions (≥ 5% of study subjects) were local infusion site reactions (such as redness, swelling, itching), headache, fever, dermatitis, asthma, diarrhea, and cough.
